Transforming the Health Care System Through Patient-Centered Care
Penobscot Community Health Care is a non-profit organization governed by a board of community volunteers, incorporated in 1997. PCHC is passionate about its mission – We provide access to patient-centered, highquality, comprehensive, integrated health care services regardless of ability to pay. Our vision is that health care is a right, not a privilege. We believe in integrated, comprehensive, evidence-based health care serving the whole person and family.

Although only ten years old, PCHC is the largest FQHC in Maine, within the top 3-4 largest and
most comprehensive of the 100 FQHCs in New England, and the largest primary care practice in
Maine. In 2009, our 120 full and part time practitioners will provide 250,000 patient visits for about
50,000 people - half the population of the greater Bangor area. Our three main clinics are open
seven days a week and most holidays – to provide continuity of care and reduce costly emergency
room visits. We provide same day sick care and provide care of new patients within 48 hours.

Quality of Care is our highest priority: All Clinics have earned 2-3 Quality Management Blue
Ribbons and PCHC is the only of the 18 FQHCs in Maine to become Accredited by the Joint
Commission. We are among the first 3-4 FQHCs in the US to have a clinic earn Certification by the
National Committee on Quality Assurance as a Patient-Centered Medical Home.
